Sutter Health is seeking an Advanced Practice Pharmacist for their California Pacific Medical Center (CPMC) in beautiful Castro, San Francisco. Sutter is leaping ahead to move their Oncology Pharmacists to the center of the model, including Medical Credentialing. The Advanced Practice Pharmacist will initiate, modify and discontinue medications within a Collaborative Practice Agreement as defined by the California State Board of Pharmacy. Focus on management of complex disease states to help patients reach optimal therapeutic goals. Simplify patient medication regimens, minimize the risk of harmful medication interactions and educate patients to enhance adherence and clinical outcomes. Responsible for policy development, Continuous process improvement, continuing education, clinical and medical staff development and Pharmacy APPE student and resident preceptorship. Performs patient care assessments, orders and interprets laboratory tests, prescribes, modifies and discontinues medication therapies. Monitors clinical outcomes, communicates with medical providers, performs electronic documentation and refers patients to other health care providers.
Meets with patients individually to provide medication support and monitoring either by 1)managing directly or 2)in partnership with physicians, advanced practice nurses, physician assistants, dietitians, medical assistants, care coordinators, and other members of the care team.
Reviews medication profile to ensure safety and proper use of medications. Alert to actual or potential medication affordability/adherence issues, interactions, therapeutic duplications, medication allergies and potential medication errors. Triages and solves medication related problems demonstrating initiative and sound judgement.
Evaluates and documents that the medication(s) selected is appropriate for achieving the desired therapeutic outcome. Selects appropriate drug formulations to achieve maximum absorption based on bariatric procedure. Communicates potential problems and likely solutions to the prescriber.
Adjusts therapy as indicated by patient condition and laboratory results.
Educates patients and their families about medications.
Communication and Education
Utilizes and communicates the appropriate prescribing of medications consistent with national clinical guidelines, evidence-based medicine, and best practices.
Communicates professional knowledge effectively in both oral and written forms to patients and other health professionals.
Develops communication materials to support clinical initiatives, as needed.
Presents clinical information to patient groups, as needed.
Participates in Medical Resident, Pharmacy Resident, and Pharmacy Student Training.
Maintains a comprehensive EPIC patient profile. The profile will include pertinent information which may influence medication therapy decisions.
Maintains a current medication list in EPIC by performing medication reconciliation at each patient visit.
Documents all patient activity in the EPIC system. Collects data, maintains logs, records and other documentation as required.
Quality and Compliance
Participates in quality improvement and medication use review activities.
Organizes, prioritizes, and plans workload within given time constraints. Works cooperatively with other members of the team to assure established objectives are accomplished.
Ensures compliance with hazardous waste collection and disposal procedures.
Maintains proficiency and expertise in area(s) of practice through continuous professional development.
Develops and maintains relationships with all customers.
Education Graduate of an accredited school of pharmacy in the U.S

Serve as a pharmacy representative for regulatory compliance surveys for inquiries dealing specifically with the verification, preparation, dispensing, storage, waste, and administration of chemotherapeutic or hazardous medications.
